3. Reference Form

Before we can process your application, we must receive a reference from your pastor. If you are under the age of 20, we need a reference from a parent or guardian* as well.

*A reference from your parent or guardian is helpful even if you are an adult or your parent/guardian is not a Christian. We occasionally allow exceptions, so please feel free to contact our office if you feel you have a reason not to provide a parent/guardian reference

4. Authorization Form

Please download the authorization form to be filled out and signed for each member of your team. Anyone under the age of 18 will need to have a parent or guardian sign the form.
